How to Apply

If you’d like to participate, please apply using the form below.
Please be sure to enter the campaign code from your campaign card in the “Requests/Questions” field.
Please enter the campaign code only when you make your first application. After that, the campaign will be automatically applied for up to five times.
*If you do not enter the campaign code, you will not be eligible for this campaign.

Important Notes

- This offer is valid for services used within 3 months of your first service date. Services provided after the 3-month period are not eligible.
- This offer cannot be combined with other promotions.
- If the total charge for a single visit is less than ¥3,000, the discount will be limited to the amount charged. Any unused portion of the discount cannot be carried over to a future visit.
